Our Mission
Uniting partners for innovative, action-oriented solutions to improve wellness and strengthen communities throughout the mountains of Eastern Kentucky.
Who We Are
A regional partnership for wellness
The Mountain Wellness Collaborative brings together the five public health departments of Kentucky's Big Sandy region with hospitals, clinics, schools, food and faith organizations, and community leaders. Together we are building a Rural Community Hub to prevent and manage chronic diseases like diabetes and obesity, and to make our five counties healthier places to live, work and grow up.
The work is funded through the Rural Health Transformation Program, and it is shaped by the partners who live and serve here.
The Work Ahead
Four areas of focus
Community of Care
Wellness coaches in every county, regional telehealth access, mobile health visits, and community exercise programs.
Food is Medicine
Food prescriptions through local clinics, nutrition education, mobile farmers markets and cooking demonstrations, and community gardens.
Data-Driven Innovation
A shared health screening, a referral system that follows each referral through to completion, and regional data to measure what's working.
Youth Prevention
Healthy-eating theater in schools, peer health ambassadors mentoring younger students, summer internships, and family wellness education.
